16. SICK AND SEVERANCE PAY <br />Sick leave is a privilege, and misuse Lnay result in disciplinary action. Sick leave may be <br />used for illness and doctor visits for the employee or the employee's dependents <br />including spouse. A dependent is defined as someone that has the same address as the <br />employee and for whom the employee provides financial support. Medical confirmations <br />by licensed medical personnel may be asked for by the Utilities. <br />Employees are required to notify their immediate supervisor at least thirty (30) minutes <br />prior to the start of their regular working hours if they intend to be absent from work. If <br />an emergency prevents the employee from notifying his,~her supervisor at such time, the <br />employee is expected to call as soon as possible during the work day. Employees are also <br />required to keep their supervisors informed of their condition and anticipated return to <br />work. <br />Sick leave may be used for up to five (5) days in a calendar month for the instances of <br />serious illness or surgery of family members who may not live with the employee. <br />Elk River Municipal Utility employees participate in the Minnesota Post Employment <br />Health Care Savings Plan (HCSP) established under Minnesota Statues, Section 352.98 <br />(Minn. Supp. 2001) and as outlined in the Minnesota State Retirement System's Trust <br />and Plan Documents. All funds collected by the employer on the behalf of the employee <br />will be deposited into the employee's post employment health care savings plan account. <br />Participation is outlined below: <br />1. Employees have agreed to contribute 1 % of gross wages to the Post <br />Employment Health Care Savings Plan. These funds will be deposited after <br />each pay period. <br />2. Employees who have accrued over 960 hours of sick time will have ~0% of <br />those hours converted to cash and deposited in their Post Employment <br />Health Care Savings account. The conversion will take place once a year at <br />the end of December. <br />3. Employees, upon. resignation, termination, or retirement, will have 50% of <br />unused sick leave, up to a maximum of 100 days, converted into cash and <br />deposited into their Post Employment Health Care Savings account. <br />17. REST PERIOD <br />Elk River Municipal Utilities will allow one hour rest time without payroll deduction for <br />each hour overtime worked from midnight to start of scheduled shift, excluding Saturday <br />and Sunday, when released. This rest time shall be taken during the next scheduled work <br />shift. When employees are not released for rest time, all hours worked on normal shift <br />shall be at time and one half. It is the employee's responsibility to notify the supervisor <br />prior to taking the rest period and to take the rest period. <br />\\ermuserver\home$\peggy\Misc.
