<br />A-2 <br />• Eligible households must first apply through the Sherburne County Emergency <br />Assistance (EA/EGA) programs when applying for emergency assistance for housing <br />or utilities. <br />• Within the time necessary to resolve the crisis, the household must be unable to <br />resolve its emergency by combining the following sources of all household members: <br />o Liquid assets & any other assets can quickly liquidate (not including vehicles) <br />o Income they anticipate receiving <br />o Tax refunds <br />o Other funds for which they are eligible <br /> <br />Any of the criteria listed above may be waived only with approval from the City’s <br />authorized representative. <br /> <br />III. Documentation Requirements. Those applying for SAHA must provide the following <br />documentation as a condition of receiving aid: <br /> <br />• Proof of income or financial hardship <br />• Proof of residency <br />• Proof of need, such as lease agreements or utility bills <br />• Person identification, such as a driver’s license, birth certificate, or other government- <br />issued ID <br /> <br />IV. Compliance and Reporting. <br /> <br />• Misuse of funds may result in disqualification from future assistance. <br />• Those receiving SAHA are required to complete and provide to Consultant a Form <br />W-9. <br />Page 66 of 322
