Minnesota State Arts Board <br />FY 2024 Arts Experiences <br />Program Overview and Application Instructions page 14 <br /> <br />In each form, applicants will select whether they are applying as an organization or individual. <br />The applicant then must either complete, save, and mark as complete; or leave blank, save, and <br />mark as complete. <br />• Form A – Organizations (Individuals must save and mark “Complete”) <br />In this form, organizations will provide information about their board and arts programming <br />administrators. When responding to the questions in this form, applicants should include <br />board members and any staff members that are in decision-making roles regarding the <br />administration of their arts programming. <br />Individual applicants will not report their information in this form, but must save it and mark it <br />complete. <br />• Form B – Individuals (Organizations must save and mark “Complete”) <br />In this form, individuals will provide information about themselves. <br />Panelists do not see this form. Individual applicants will be asked to describe themselves, <br />the communities they are part of, and the communities they serve elsewhere in the <br />application. Applicants that are not U. S. citizens, but have permanent resident status, will <br />be directed to attach a copy of their permanent resident card as a PDF file. <br />Organization applicants will not report their information in this form, but must save it and <br />mark it complete. <br />Nonprofit and Fiscal Sponsor Form <br />Provide evidence and documentation related to the applicant’s eligible entity type. <br />Fiscal sponsor materials <br />An applicant must apply with a fiscal sponsor if it is an arts group without 501(c)(3) status, or an <br />individual requesting more than $25,000. <br />Applicants who are required to use a fiscal sponsor must submit the following: <br />• The fiscal sponsor agreement, see below (attach as PDF file). <br />• The fiscal sponsor’s IRS 501(c)(3) determination letter (attach as PDF file). If the fiscal <br />sponsor is a public entity, such as a unit of state, local, or tribal government, or a public <br />school, no documentation is required. <br />• The fiscal sponsor’s legal name and contact information. <br /> <br />Fiscal sponsor agreement <br />Applicants who are required to use a fiscal sponsor must submit a fiscal sponsor written <br />agreement that: <br />• Includes a description of the relevant responsibilities of both the applicant and the fiscal <br />sponsor, <br />• Is signed by both the applicant and the fiscal sponsor, and <br />• Is dated within 12 months of the application deadline. <br /> <br />The following are not allowed in this program <br />• A nonprofit organization or public entity cannot serve as fiscal sponsor for an individual who <br />is an employee, contractor, or board member of the nonprofit organization or public entity. <br />• A nonprofit organization or public entity cannot serve as fiscal sponsor for a group that
