Section 2 <br />Proposal Requirements and Conditions <br />2.10 Prequalification of Proposers <br />All proposers are required to submit at least three (3) referrals for whom the proposer provides services to a <br />similar client as the city, i.e., providing the same services to a similarly used facility which has an equivalent <br />amount of square feet as the Elk River contract. Please include the references as part of the proposal. Also <br />include an Affidavit of Non -Collusion and Certificate of Acknowledgement. <br />2.20 Contents of Proposal Form <br />The proposal form shall state the location and description of the services, the amount and nature of the <br />guaranty deposit required if any and the date the proposals will be due. <br />Bound within the proposal forms will be any Special Provisions and other supplementary requirements. All <br />papers bound with or attached to the proposal form are essential parts of the proposal and shall not be <br />detached from or altered without specific authorization. The plans, specifications and other documents <br />designated in the proposal are also a part thereof, whether attached or not. <br />2.30 Specifications and Site of Work <br />The proposer is expected to carefully examine the sites of the proposed work, the specifications, and the <br />proposal and contract forms, before submitting a proposal. The submission of a proposal shall be <br />conclusive evidence that the proposer has investigated and is satisfied as to the conditions to be <br />encountered in performing the work as to the requirements of the specifications and contract. <br />2.40 Preparation of Proposal <br />The proposer shall submit his or her proposal upon the forms furnished by the city. All figures shall be in <br />ink or typed. <br />The proposer's proposal shall be signed with ink by the individual, by one or more members of the <br />partnership, by one or more officers of each firm representing a joint venture or by one or more officers of <br />a corporation. If the proposal is made by an individual his or her name and post office address shall be <br />shown; by a partnership the name and post office address of each partnership member shall be shown; as a <br />joint venture the name and post office address of each member officer of the firms represented by the joint <br />venture shall be shown; by a corporation the state in which it was chartered, and the business of its <br />corporate officials shall be shown. <br />2.50 Delivery of Proposals <br />Each proposal shall be submitted via QuestCDN. <br />13 <br />
