The eligibility requirements and benefits <br />provided, if any, shall be specified in materials provided by the <br />respective insurance carriers. The respective carriers may have <br />different requirements concerning the eligibility of employees. <br />The requirements of the respective carrier will govern eligibility. <br />The City may change insurance carriers and programs at any time in <br />its sole discretion. Any such change may affect eligibility <br />requirements and/or benefit limits under any given insurance <br />policy. <br /> <br />10. Performance Appraisals. <br /> <br />a. Introduction - Performance appraisals are an <br />opportunity for employees, supervisors, and the City <br />to assess an individual's job performance. A system <br />for appraising performance of employees may be <br />developed and placed into effect with the approval of <br />the City Administrator. The performance appraisal <br />system shall be based on an employee's job duties. <br />It shall be designed to access individual performance <br />and encourage employees to reach their maximum <br />potential so that services provided to the City are <br />enhanced. Performance appraisals shall be conducted <br />on an annual basis or more frequently if prescribed <br />by the City Administrator, an employee's department <br />head, or this Personnel Ordinance. <br /> <br />b. <br /> <br />The following guidelines may be included in the <br />appraisal system: <br /> <br />(i) An employee's immediate supervisor will <br />conduct a performance appraisal of the <br />employee on an annual basis. For purposes of <br />this provision, an immediate supervisor is <br />that individual who assigns, checks, inspects <br />and/or directs the employee's work. The <br />supervisor is also generally responsible for <br />the employee's discipline. <br /> <br />(ii) Performance appraisals shall be in writing and <br />signed by the employee and the supervisor. u <br /> <br />(iii) An official copy of the most recent <br />performance appraisal shall be kept on file in <br />the City Administrator's Office and by each <br />department if required by the City <br />Administrator. <br /> <br />(iv) Performance appraisals shall be based on job <br />responsibilities and job-related performance <br />standards. <br /> <br />(v) The performance appraisal shall be used for <br />identifying problem areas in an employee's <br /> <br />19. <br />
