CHANGE ORDER No. 1 PROJECT: 2021 Street Improvements Project DATE OF ISSUANCE: OWNER: City of Elk River ENGINEER'S Project No.: R13.121582 CONTRACTOR: North Valley, Inc. EFFECTIVE DATE: ENGINEER: Bolton & Menk, Inc. You are directed to make the following changes in the Contract Documents. Description: CO 1.1— Watermain Valve & Structure Chimney Seal Modification 1. ADD the attached pay items as CO- 1.1 at the unit prices listed to the contract documents: CO 1.1—Watermain Valve & Structure Chimney Seal Modification- Construction Cost = $75,134.72 CO 1.2 — Woodland Trails Park Improvements 1. ADD the attached pay items as CO-1.2 at the unit prices listed to the contract documents: CO 1.2 —Woodland Trails Park Improvements- Construction Cost = $130,515.27 CO 1.3 — Twin Lakes Road Turn Lane Modification 1. ADD the attached pay items as CO-1.3 at the unit prices listed to the contract documents: CO 1.3 — Twin Lakes Road Turn Lane Modification- Construction Cost = $60,500.42 CO 1— Specification 1. ADD the attached specification items to the contract documents. CO 1— Plan Sheets 1. ADD the attached revised plan sheets to the contract documents. Reason for Change Order: CO 1.1— Watermain Valve & Structure Chimney Seal Modification: ERMU has indicated that some of the project impacted valve boxes should be replaced and that an unused watermain valve shall be cut out and the main capped. The impacted gate valves & boxes are on Jackson Avenue NW, 1791' Avenue NW, Zane Street NW, Industrial Circle NW, 171St Avenue NW, and Twin Lakes Road NW. Exterior pipe grout repair will be performed on a catch basin on the northwest quadrant of Jackson Avenue NW and 1901h Avenue NW. All project impacted manholes and catch basins will be fitted with a chimney seal to reduce water infiltration. The pay items in CO- 1.1 are needed to complete these modifications. The attached unit prices are hereby approved in the total amount of $75,134.72. CO 1.2 Woodland Trails Park Improvements: The City intends to install a 10' wide bituminous trail the walk on the east side of Elk Lake Road NW to an existing trail within the Woodland Trails Park. This project also includes a 1.5" overlay on the park access road and a 1.5" mill & overlay on the parking lot. The pay items in CO-1.2 are needed to complete this project. The attached unit prices are hereby approved in the total amount of $130,515.27.
