Project outcomes describe measurable and achievable change in knowledge, attitude, skill, behavior, or <br />condition among the people the project is designed to 
benefit. <br />2. The evaluation plan includes appropriate methods to measure and document project
 completion, <br />effectiveness, and progress made toward project outcome(s). <br />3. The evaluation plan identifies how evaluation results will be used to improve current or
 future <br />programming (e.g., adjustments to the project, informing future programming, sharing with stakeholders <br />or peers, applicant learning and development, etc.).
