Admission Criteria <br />Admission Criteria: Individuals considered for placement must have a diagnosis of a serious <br />and persistent mental illness by a Mental Health Professional. No individual is discriminated <br />against in admission, provision of services, or termination because of race, color, national <br />origin, religion, gender, physical handicap, sexual preference, marital status, or public <br />assistance status. <br />Applicants seeking admission to the residential treatment/rehabilitation program will meet <br />some or all the following criteria: <br />1) Individual has a mental illness as defined in comprehensive mental health act MINN. <br />Statues 245.461. Subd.20. (a): <br />2) Those on a provisional discharge or stay of commitment who have the potential of self -harm <br />may be appropriate for this level of service if the program determines this service provides <br />adequate structure and there are sufficient support systems in place in the community and <br />safety of others is considered; <br />3) Individual can reasonably be expected to commence or resume illness management and recovery <br />skills/strategies at this level of service and needs 24-hour supervised and monitored and focused <br />treatment approach to accomplish this improvement in functioning and avoid relapse requiring a <br />higher level of care; <br />5) Individual symptoms are difficult to manage, or clearly inappropriate for, active treatments at a lesser <br />level of care; <br />6) Individual needs a restrictive setting and is at risk of significant functional deterioration if intensive <br />residential treatment services are not received. <br />7) Individual has one or more of the following: <br />• A history reoccurring or prolonged inpatient hospitalizations in the past year <br />Significant independent living instability <br />• Homelessness <br />• Very frequent use of mental health and related services yielding poor outcomes in outpatient <br />community support treatment <br />8) Individual, in the written opinion of a licensed mental health professional and in consultation with <br />the mental health case manager, has the need for mental health services that cannot be met with <br />other available community-based services or the individual is likely to experience a mental health <br />crisis or require a more restrictive setting if intensive rehabilitative mental health services are not <br />provided <br />9) Applicants must not need specialized diet and/or nursing care. <br />10) Review of staffing patterns and competencies of staff result in a determination that the program can <br />meet the needs of a person seeking admission <br />Other Diaanostic Cateaories and Classifications we Will Acceot that are Secondary to Mental Illness <br />1) We will take clients who have a secondary diagnosis of chemical dependency as long as they <br />are appropriate to the program. <br />2) We have taken people who have a secondary diagnosis of different personality disorders <br />depending on the client mix and our ability to work with them <br />3) We may take some people who have a mild TBI secondary to mental illness depending the <br />client's ability to gain from the program. <br />4) According to policy at the Minnesota Department of Human Services we cannot take level 3 <br />sex offenders. We may take a level 1 or 2 if we believe the person will benefit from the <br />program and have a history of functioning well in the community. We try to consider the current <br />mix of clients and the community they live in. It is important to note that mental Illness needs to <br />be the primary reason for their presence in the facility <br />
