<br />. <br /> <br />. <br /> <br />. <br /> <br />December, 1993 <br /> <br />5 of 8 Pages <br /> <br />PROGRAM: Wellness <br /> <br />WAGE INCENTIVE: 100/0 of the total available <br /> <br />COMPONENTS: Wellness - In order to obtain the wage incentive <br />available from the voluntary wellness section of this <br />program, the employee will have to verify that hel she <br />has not used tobacco for the last twelve months. If an <br />employee has stopped the use of tobacco at some point <br />in the year and has continued to not use tobacco <br />through the end of the year, the Committee appointed <br />by the Chief of Police to administer the WIP may <br />recommend to the Chief of Police partial payment for <br />this wellness section of the WIP. There are no tests <br />that are required for this section of the program. It is <br />acknowledged that this section of the program is <br />voluntary and somewhat on the "honor" of the <br />employee. <br />
