Also, it is a review of <br />OSHA and DOT Safety compliance of your program. The SP-9 evaluation format that I <br />am using is detailed in your AWAIR Program <br />Employer Commitment: <br />• Very Pro-active. Safety is a concern. <br />2. Employee Involvement: <br />• Good attitude. Good participation. <br />• The City Safety Committee tries to be very Pro-active and the Utility <br />representation is a great asset. <br />3. Hazard Control: <br />• Documented walk through self-inspections are done on a regular basis. <br />4. Facility Maintenance: <br />• Concerns are addressed as needed. <br />5. Accident Analysis: <br />• Accidents are being reviewed. <br />• Near misses are identified and reviewed. <br />6. Occupational Medicine: <br />• Complete as needed. <br />7. Contractor Safety: <br />• Continue to inform contractors of their Safety responsibilities. <br />8. Employee Training: <br />• Compliance Training is on pace and documented. <br />9.
