All employees and managers <br />must follow the regulations and laws of the State of Minnesota and rules of the <br />i~i~ tilities governing the safety of employees and the public. [f employees have <br />questions or concerns about issues affecting safety, they should immediately discuss them <br />with their supervisor, any manager, or the relevant public safety authority. <br />Employees are required to report accidents resulting in personal injuries and/or vehicle, <br />equipment, or property damage to their supervisor immediately. <br />- Personal Injuries. Immediately report to your supervisor all accidents <br />and injuries occurring within the course of~ your employment. The <br />supervisor shall submit a First Report of Injury and a Supervisor's <br />Report of Injury Form to the I-Iuman Resources Representative within <br />twenty-four (24) hours of the time of injury. <br />- Vehicle, equipment, or property damage accidents. Immediately <br />report to your supervisor all damage to ~i~-ity tiliti property. The <br />supervisor should submit documentation regarding the incident to the <br />tiliti 'Office Manager within twenty-four (24) hours of the time of <br />the damage or accident. A copy of the Minnesota Motor Vehicle <br />Accident Report must be submitted for all vehicle accidents. <br />The 7 Tacci'nJ tilitie will provide necessary safety equipment to employees. This may <br />include: <br />One pair of safety glasses at the time of employment. The employee shall <br />provide the correct prescription for the safety glasses. <br />The ~ tiliti s will pay the cost of new safety lenses, upon a change in <br />the prescription. If needed, frames will be replaced every four years. <br />Safety glasses that are broken or damaged while the employee is on the job <br />will be replaced by the i r tilitie <br />~9:.L>L, EXPOSURE TO HAZARDOUS SUBSTANCES <br />Any employee routinely exposed to hazardous substances or harmful physical agents as <br />defined in the Minnesota Employee Right to Know Act (Mr[v~t. STAT. § 182.675) shall be <br />trained before being assigned or reassigned work exposing the employee to such <br />substances or agents and shall be given training annually thereafter.
