The employee must respond to such a request within 15 days of the request or <br />provide a reasonable explanation for the delay. Failure to provide certification may result <br />in a denial of continuation of leave. This certification will be provided using the DOL <br />Certification of Qualifying Exigency for Military Family Leave. <br /> <br />(s) Certification for Serious Injury or Illness of Covered Servicemember for <br />Military Family Leave <br />The Utilities will require certification for the serious injury or illness of the covered <br />servicemember. The employee must respond to such a request within 15 days of the <br />request or provide a reasonable explanation for the delay. Failure to provide certification <br />may result in a denial of continuation of leave. This certification will be provided using <br />the DOL Certification for Serious Injury or Illness of Covered Servicemember. <br /> <br />All other provisions of the FMLA policy, including Use of Paid Leave, Employee Status <br />and Benefits During Leave, Procedure for Requesting Leave, and Benefits During Leave <br />and Reinstatement, are outlined above in the FMLA policy. <br /> <br /> <br />55. PARENTING LEAVE <br /> <br />Under the Minnesota Parental Leave Act, a Minnesota employee who has worked for the <br />Utilities for at least 12 months and who has worked at least one-half (1/2) time during the <br />twelve (12) months preceding a leave is entitled to take up to twelve (12) weeks of unpaid <br />leave as follows: <br />• An eligible biological or adoptive parent make take such leave for the birth or <br />adoption of a child; and <br />• An eligible female employee may take such leave for prenatal care or incapacity <br />due to pregnancy, childbirth, or related health conditions. <br />The length of parenting leave under this section shall not be reduced by any period of paid <br />or unpaid leave taken for prenatal care medical appointments. <br />An employee requesting parental leave must give the Utilities at least thirty (30) days <br />advance notice of the start and end dates of the requested leave, unless such notice is not <br />possible due to legitimate unanticipated factors in which case as much notice as is possible <br />should be given. For a leave for the birth or adoption of a child, the leave must begin within <br />either 12 months of the birth or adoption or, if the child must remain in the hospital longer <br />than the mother, within 12 months of the child leaving the hospital. <br />Insurance benefits will continue during the leave, but the employee must pay their portion <br />of the premiums during the leave and will be required to reimburse the Company for <br />premiums paid by the Company during the leave if the employee does not return to work <br />following the leave. <br />130
