o child, foster child, adult child, legal ward, child for whom the em�lo�ree is legal <br />guardian, or child to whom the em�lo�ee stands or stood in loco �arentis; <br />o s�ouse or registered domestic �artner; <br />o sibling, ste�sibling, or foster siblin�; <br />o biolo�ical, adoptive, or foster �arent, ste�arent, or a�erson who stood in loco <br />oarentis when the em�lo�ee was a minor child; <br />o grandchild, foster grandchild, or ste�grandchild; <br />o grand�arent or steggrand�arent; <br />o a child of a sibling of the em�lo�-ee (niece or ne�hew�; <br />o a sibling of the �arents of the em�lo�ree �aunt or uncle� or <br />o a child-in-law or siblin -in-law; <br />■ a s�ouse or registered domestic �artner's famil� members as listed above; <br />■ an�r other individual related b�r blood or whose close association with the em�lovee is the <br />eauivalent of a familv relationshi�; and <br />■ up to one individual annuall� designated bv the em�lovee. <br />Rec�uired Notices <br />If the need for the use of ESST is foreseeable, the em�lo�ree must �rovide notice at least seven (� da�s in <br />adv�u�ce of the use of the leave. Notice shall be �rovided b�� mal�ing the rec�uest through the em�lo`�ee's <br />time management or HRIS s,rs� <br />If the need is unforeseeable, the em�lo�ree must �rovide notice as soon as �racticable. If �ossible, <br />em�lo�rees must notifv their immediate su�ervisor at least thisttr �30) minutes before the start of their <br />regular start time if s/he intends to be absent from work. If an emergenc�� �revents the em�lo��ee from <br />notif�rin,g the immediate su�ervisor, s/he must contact the immediate su�ervisor as soon as �ossible <br />during the workda�r. Em�lo`rees are rec�uired to kee� their immediate su�ervisor informed of their <br />antici�ated return to work. Em�lo�rees shall follow notice rec�uirements for the use of sick time set forth <br />in union contract if different from the rec�uirements abo�re. Em�lo`ree is res�onsible to ensure sick time <br />usa�e is recorded in the timekee�ing s�rstem as soon as �racticable but no later than the end of the �a�r <br />neriod in ��hich the time was taken. <br />Time off rec�uests must include who the time is being used for, their relation to the em�lo�ree, and the <br />reason for the use. <br />���en an em�lovee uses ESST for more than three consecutive da,-�s the em�lo�ree shall �rovide <br />apn�o�riate su�orting documentation (such as medical documentation su�orting medical leave, <br />court records or related documentation to su�ort safet�-leave). However, if the em�lo�-ee or <br />em�lo�ree's f�unilv member did not receive services from a health care �rofessional, or if <br />documentation cannot be obtained from a health care �rofessional in a reasonable time or without <br />added ex�ense, then reasonable documentation mav include a written statement from the em�lovee <br />indicating that the em�lo�ree is using, or used, ESST for a c�ualif,�ing �ur�ose. The cit�r will not rec�uire <br />an em�lovee to disclose details related to domestic abuse, sexual assault, or stalking or the details of the <br />em�lo�ree's or the em�lo�-ee's famil`r member's medical condition. <br />In accordance with state law, the cit�� will not reauire an em�lo�-ee using ESST to find a re�lacement <br />«-orker to cover the hours the em�lo��ee will be absent. <br />Carr� Over of ESST <br />Em�lo�-ees will carr;r over accrued but unused ESST into the following calendar �ear.
