For example, if an applicant’s <br />fiscal year runs October – September, the period ending September 2021 would be FY 2021. <br /> <br />File naming requirement <br />• Name files with a combination of the applicant name and document title. For example: <br />YourOrganizationName-Financial2021.pdf, or <br /> YourOrganizationName-501c3-Determination.pdf <br />• Use only letters of the alphabet, numerals, underscores, or dashes in file names. Do not use <br />commas, periods, other punctuation marks, or special characters, as these may result in file <br />corruption. <br />Populations Benefitting Form <br />Estimate the percentage of people engaged by the applicant’s arts programming in calendar <br />year 2022, who identify with each of the listed demographic groups. These responses should <br />refer to populations engaged directly; only include broadcast or online audiences and <br />participants that were directly engaged in arts programming (such as viewers who downloaded <br />a video performance or viewed an online exhibition). Do not report people who engaged in your <br />electronic marketing efforts (such as people who saw a social media post about a festival or <br />visited your website to purchase tickets to a concert). <br />• Age – An individual should be reported in only one group. Percentages should add up to <br />100. <br />• Geography – An individual should be reported in only one group. Percentages should add <br />up to 100. <br />• Race/ethnicity – An individual should be reported in only one group. Percentages should <br />Organization with FY 2021 annual <br />revenue of $50,000 – $750,000 <br />IRS form 990 or 990-EZ or certified audit if available. If your <br />organization’s fiscal year begins on January 1, attach, the IRS <br />990 dated 2021. If your organization’s fiscal year begins in any <br />other month other than January, attach the IRS 990 dated <br />2020. <br />Organization with FY 2021 annual <br />revenue less than $50,000 <br />Internal board approved financial statements for fiscal year <br />2021, which must include the statement of financial activities <br />(income and expense) and statement of financial position <br />(balance sheet) for fiscal year 2021. <br />Arts affiliates Financial statements for fiscal year 2021 that show the <br />affiliate’s statement of financial activities (income and expense <br />statement) separate from the host institution’s. <br />If the arts affiliate does not have its own statement of financial <br />position (a balance sheet), include the host institution’s fiscal <br />year 2021 statement of financial position (balance sheet) <br />along with the affiliate’s fiscal year 2021 statement of financial <br />activities (income and expense statement). <br />Arts groups without 501(c)(3) <br />status <br />Financial statements for fiscal year 2021, which must include <br />the statement of financial activities (income and expense <br />statement) and statement of financial position (balance sheet) <br />for fiscal year 2021.
