Date shall be no more than ten business days after the deadline for donations to the <br />receiving employee. <br /> <br />6. Before the Donation Effective Date, if Paid Leave Donation Forms are submitted <br />for a total amount of leave that is more than the number of hours requested by the <br />employee in the Paid Leave Donation Request Form, or for more than the <br />maximum total donation of 240 hours, the Utilities will pro rate donations to the <br />maximum allowed, among all employees submitting a Paid Leave Donation Form <br />for the receiving employee and will notify all such employees in writing of such <br />pro rating and the final number of hours from their Paid Leave Donation Form that <br />will be donated to the receiving employee on the Donation Effective Date. <br /> <br />7. On the Donation Effective Date the Utilities will transfer all paid leave donations <br />that are indicated collectively on the Paid Leave Donations Forms submitted, <br />prorated, if necessary, in accordance with this policy, to the account of the <br />receiving employee; and will reduce the corresponding account balances of the <br />donating employees accordingly. <br /> <br />8. Donation of paid leave is permanent and final as of the Donation Effective Date <br />and cannot be rescinded thereafter. Donated paid leave may not be transferred <br />back to the donor after the Donation Effective Date under any circumstances. If <br />the donated paid leave is not used by the receiving employee it is forfeited by all <br />parties. <br /> <br />9. Donated paid leave time may be used by the receiving employee only for purposes <br />of the medical leave necessitated by the catastrophic life event supporting leave <br />donation under this policy. <br /> <br />10. Donated paid leave may be used by the receiving employee only for work time <br />actually and necessarily missed due to the leave taken for the purpose set forth in <br />the leave request submitted under this policy, and for no other purpose. <br /> <br />11. Donated paid leave may be used by the receiving employee only to replace the <br />employee’s normal work hours lost, up to a maximum of the receiving employee’s <br />FTE (measured as of the date on which the leave begins). <br /> <br />12. Donated paid leave must be used by the receiving employee concurrently with any <br />applicable unpaid leave available to the receiving employee. <br /> <br />13. Donated paid leave must be used during the leave taken for the purpose submitted <br />in the leave request under this policy. <br /> <br />14. Donated paid leave not used as described in this policy will be forfeited by the <br />recipient. <br /> <br />147
