<br />(4) A list of all parties who control or own an interest ~n excess of <br /> <br />five percent ~n such corporation or organization and all information <br /> <br />concerning said person as is required in subparagraph b above. <br /> <br />(e) Whether the applicant is licensed ~n other communities to run similar <br /> <br />businesses; and if so, where. <br /> <br />(f) Whether the applicant has previously been denied a conversation parlor <br /> <br />license, or has had a license revoked. If so, g~ve details of each. <br /> <br />(g) The names, residences, and business addresses of three residents of the <br /> <br />metropolitan area, of good moral character, not related to the applicant <br /> <br />or financially interested in the premises or business, who may be referred <br /> <br />to as to the applicant's or manager's character. <br /> <br />(h) A description and location of the premises to be licensed. If the <br /> <br />premises are not constructed and furnished at the time the application <br /> <br />is filed, the detailed plans of the premises and furnishings shall be <br /> <br />filed with the application. <br /> <br />(i) A description of the services to be provided. <br /> <br />(j) List of types of equipment to be used in the business. <br /> <br />(k) Such other information as the Council may requ~re. <br /> <br />20.04 Review of License Application and Council Action. <br /> <br />(a) All license applications shalll be referred to the Chief of Police and <br /> <br />such other city departments as the City Administrator deems necessary <br />\ <br />for verification and investigation of the facts set forth in this <br /> <br />applicat~on. The Chief of Police and other department heads consulted <br /> <br />shall submit their reports and recommendations to the City Council. <br /> <br />(b) The City Council, upon receipt of the recommendations required in <br /> <br />subparagraph (a), shall afford the applicant and all interested parties <br /> <br />a public hearing and shall consider the standards contained in Section <br /> <br />4. <br />
