(j)Assessment ofthe effectiveness of active landfill gas control <br />operations based on the results of groundwater and landfill gas <br />monitoring and incorporate the evaluation in the MPCA Annual <br />Report for the landfill. <br />(k)Ahigh resolutionengineering plan to fully illustrate all facility <br />features reviewed in the annual report (landfill gas probes, <br />monitoring wells, gas collection infrastructure, leachate <br />recirculation infrastructure, etc.). The Plan must identify permitted <br />and remaining volumes <br />(l)Information concerning the pressure jetting of the leachate <br />collection system. ERL must provide a discussion of any major <br />maintenance activitiesor corrective actions performedrelated to <br />leachate collection pipe blockages inthe Annual Report. <br />(m)Alist of rejected waste loads and summary with the Annual <br />Operating Report. A summary and analysis of random load <br />inspections must be provided with the Annual Operating Report. <br />(08)Closure, Postclosure and Contingency Action Plan”, March 2012 <br />The Licensee shall revise the Plan to account for the current conditions of <br />the Site. The contingency action portion of the Plan must reflect the potential <br />failure of monitored natural attenuation and the need to implement active <br />groundwater remediation of contaminated groundwater adjacent to landfill within <br />the compliance boundary. Costs associated with groundwater remediation <br />contingenciesmust be increased to address the likelihoodof active groundwater <br />remediation.A revised Planshall be submitted within 6 months of the effective <br />date of the current License. <br />(09)Construction Quality Assurance <br />A Construction Quality Assurance (CQA) Report shall be submitted to the <br />City prior to the placement of waste on newly constructed lined areas. A CQA <br />report shall also be submitted for Landfill areas which receive final cover. A CQA <br />report shall also be submitted for installation of leachate recirculation drainfield <br />areas to document position within the cell, suitable basecontours and piping <br />elevations. The report shall demonstrate that the work was completed to the plan <br />specifications. The Report shall be certified by an engineer registered in the State <br />of Minnesota and shall include but not be limited to: <br />(a)All relevant construction specifications; <br />(b)All shop drawings for prefabricated components; <br />(c)Photo documentation of all critical aspects of construction; <br />10 <br /> <br />
