The Bidder shall acknowledge receipt of this Addendum in the space provided on the Bid Form. Failure to do so <br />may subject the Bidder to disqualification. <br />This Addendum consists of a total of (786) pages which includes this narrative, and related attachments. <br />1) GENERAL INFORMATION <br />a) Questions & Answers <br />1. Question #1: Are combined bids for more than one bid category acceptable? <br />(1) Answer #1: No. Please submit each bid for one corresponding bid category. If you wish to bid multiple bid <br />categories, submit a separate bid in a sealed envelope for each individual bid category you choose to bid. <br />2. Question #2: Is this project Tax Exempt? <br />(1) Answer #2: No. Taxes are to be included in the base bid. However there is a line item on the bid form to <br />identify the amount of taxes that are included in the base bid. <br />3. Question #3: Are prevailing wages required? <br />(1) Answer #3: No <br />4. Question #4: Who will be responsible for construction staking & surveying? <br />(1) Answer #4: The Construction Manager will retain the services of a professional surveyor to complete the <br />construction staking and surveying required. <br />5.
