<br /> <br /> 43 <br />January 2020 <br />Summary Plan Description for details regarding coverage and eligibility. A copy of this <br />description will be provided to employees and is available upon request. <br /> <br /> <br />49. VISION INSURANCE <br /> <br />Eligible employees who regularly work at least 30 hours per week are eligible to enroll in <br />the Utilities’ vison insurance plan. The monthly premium for vision coverage is the <br />responsibility of the employee. Employees should consult the Summary Plan Description <br />for details regarding coverage and eligibility. A copy of this description will be provided <br />to employees and is available upon request. <br /> <br /> <br />50. HOME COMPUTER LOAN POLICY <br /> <br />In an effort to encourage all employees to develop and improve their computer skills, <br />employees may purchase a home computer per the following guidelines. <br />1. The purchased computer and software shall be new, not reconditioned, not used. <br />2. The purchased computer shall be used in the employee’s home. <br />3. Employee must provide a receipt reflecting the purchase of this computer. <br />4. The Utilities will provide an interest-free loan for the purchase of this computer up <br />to $1,200.00, for a term not to exceed 12 months. <br />5. Employee will repay the computer loan to the Utilities in not more than 25 equal <br />installment payments authorized and made through payroll deduction, upon the <br />employee’s election by signed authorization form. <br />6. Employee must maintain ownership and possession of the computer for the <br />duration of the loan term. <br />7. If the employee leaves the Utilities before the loan is paid back in full, the Utilities <br />will withhold the outstanding portion of the loan from the employee’s final <br />paychecks (including any check for unused and unpaid benefits) as authorized by <br />the employee in the authorization form signed at the time the loan is made. <br /> <br /> <br />51. EDUCATIONAL ASSISTANCE <br /> <br />The Utilities will provide financial assistance to eligible employees for pre-approved <br />courses that are job related, lead to a job-related degree, or are within areas beneficial to <br />the Utilities in its discretion and pursuant to this policy. With regard to education required <br />by the Utilities or in connection with a Utilities-approved apprenticeship training program, <br />check with a Utilities manager or Director. <br /> <br />1. All full-time Utilities employees with at least three months of continuous <br />employment are potentially eligible for reimbursement under this policy. <br /> <br />2. The Utilities may pay up to 100% reimbursement of the costs for tuition, <br />registration, fees, books and course required materials after successful completion <br />130
