Location of premises: <br /> Address <br /> Lot Block Addition <br /> 2. Installation shall commence upon receipt by ERMU from the Owner of the following: <br /> A) Notice that the installation of the electrical service entrance has been completed. <br /> B) Must prepay the temporary electric service fee per ERMU fee schedule, plus tax. The total amount due for <br /> each installation shall be due prior to the hook-up of the temporary electric service installed by ERMU. <br /> 3. The prepaid temporary electric service fee, per ERMU fee schedule,includes installation and removal of <br /> temporary electric service. <br /> 4. The prepaid temporary electric service fee, per ERMU fee schedule,does not include electric usage. Electric <br /> usage will be charged to the Owner at the current electric rate. <br /> 5. The electric line installed by the Utilities shall remain the property of ERMU and the payment by the Owner shall <br /> not entitle Owner to ownership interest or rights therein. <br /> 6.
