Recipientsmay <br />access andreceiveservices fromtheprogram outside ofthe facilitywhenitwouldbenefitthe <br />continuityoftreatmentandtransition tothecommunity. Thefollowingservices mustbeavailable <br />andoffered aspartoftheprogram design: <br />Supervision anddirection <br />Individualized assessment andtreatmentplanning <br />Crisis assistance, developmentofhealth caredirectivesandcrisis preventionplans <br />Nursingservices <br />Interagency casecoordination <br />Transition anddischargeplanning <br />Livingskillsdevelopment, including: <br />Medication self-administration <br />Healthyliving
