descriptionFor more information about the Utilities' benefit plans consult the summary <br /> plan descriptiondescriptions that have been distributed to each employee, additional <br /> copies of which are also available upon request. <br /> 42. HEALTH INSURANCE COVERAGE <br /> For eligible employees who regularly work 30 hours per week, and enroll in a health <br /> insurance plan provided by the Utilities, the Utilities will pay seventy-five percent (75%) <br /> of the monthly premium per employee/family for health insurance. The Utilities currently <br /> offers both a conventional plan and a Health Savings Account-related plan. Plans and plan <br /> designs may change from time to time. The amount of the monthly premium for a health <br /> insurance plan over and above 75%of the total monthly premium for health care coverage <br /> is the responsibility of the employee. Employees should consult the Summary Plan <br /> Descriptions of the available plans for details regarding coverage and eligibility. A copy of <br /> these descriptions will be provided to employees and is available upon request. <br /> 43. DENTAL INSURANCE <br /> Eligible employees who regularly work at least 30 hours per week and who enroll in the <br /> Utilities' dental insurance plan, the Utilities will pay seventy-five percent (75%) of the <br /> monthly premium per employee/family for dental insurance. Any monthly premium over <br /> and above 75% of the dental coverage is the responsibility of the employee. Employees <br /> should consult the Summary Plan Description for details regarding coverage and <br /> eligibility. A copy of this description will be provided to employees and is available upon <br /> request. <br /> 44. LONG-TERM DISABILITY <br /> A Long-Term Disability Plan is provided to eligible employees who regularly work at least <br /> 30 hours per week. Schedule amount: 6-64460% of Monthly Earnings subject to a <br /> maximum amount of $5,000.00 per month. Employees must be employed for two (2) <br /> months to qualify for coverage. Employees should consult the Summary Plan Description <br /> for details regarding coverage and eligibility. A copy of this description will be provided <br /> to employees and is available upon request. <br /> 45. LONG TERM CARE INSURANCE <br /> Long Term Care Insurance is available to employees, at the employee's election and <br /> expense, upon application and approval by the insurance provider. Employees should <br /> consult the Summary Plan Description for details regarding coverage and eligibility.
