<br />. <br /> <br />. <br /> <br />. <br /> <br />.. <br /> <br />GRANT FUNDING: During the two year process of the Healthy Communities <br />Partnership Program, a grant of up to $10,000 will be available to each community to <br />carry out the work required. Expenses may include travel expenses to the training <br />sessions, the cost of facilitators, food and space for community sessions, etc. <br /> <br />REQUIREMENTS: To apply for the Program, a community should have a group of <br />citizens who are committed to planning effectively for the future of the community. This <br />leadership team should represent as many aspects of community life, as possible, and <br />there should be a willingness to include young people, as well as seniors. The leadership <br />team must commit to having a minimum of ten participants at the six training sessions <br />over a period of one year. They should also be willing to recruit additional community <br />members to participate, coordinate a visioning and planning process, and identify <br />implementation projects. <br /> <br />PROCESS: Communities should apply for the program by submitting a written <br />application using the Fund's guidelines. Ifinitial criteria are met, the community will be <br />asked to host a site visit. At the site visit, Initiative Fund representatives will explain the <br />program in more detail and explore whether this is the right option for the community. <br />After the site visit and review of the application, the Initiative Fund will determine <br />acceptance into the Program.
