F. Insurance Requirements—The contractor shall provide the city with proof of the following <br /> insurance with the submittal of this quote: <br /> a. Worker's Compensation Insurance Coverage in accordance with Minnesota State <br /> Statutes. <br /> b. Comprehensive General Liability Insurance with a minimum combined single limit of <br /> $1,000,000 for each occurrence for bodily injury and property damage. <br /> c. Automobile Liability Insurance covering all owned,hired,and non-owned vehicles, <br /> including residual liability insurance with a minimum combined single limit of$1,000,000 <br /> for each accident for bodily injury and property damage. <br /> d. In the event of an accident of any kind,the contractor shall notify the city immediately <br /> and furnish the city with copies of all reports of such accidents within 48 hours. <br /> G. Termination of Contract <br /> a. The city may,by written notice of default to the successful bidder,terminate the whole <br /> or part of the contract in any one of the following circumstances: <br /> i. If the contractor fails to perform the services within the time specified herein or <br /> any extension thereof;or fails to perform the services at the price accepted by <br /> the city; <br /> ii. If it is determined that the successful bidder knowingly falsified information <br /> provided to the city; <br /> iii. Payment may be withheld if work is not performed as described under items A-E <br /> of this request;and the termination of this contract may occur. <br /> Responsible bidders shall provide a completed quote tabulation form(attached)to include a price <br /> per week for mowing services,price per application of fertilizer and weed control,and a price for <br /> aeration. Contractor shall submit three letters of reference for similar work conducted for other <br /> government entities and/or private businesses. <br /> Quotes are due to the City of Elk River no later than December 21,2016.
