Introduction <br /> <br />A.I. Project <br />The City of Elk River is planning to improve various streets within the city. A shown in Figure 1 <br />in the Appendix, the main roads being improved include Upland Avenue from Highway 10 to <br />Main Street, Tipton Avenue from Upland Avenue to Main Street and 5th Street from one block <br />west of Seymour Avenue to Norfolk Avenue. <br /> <br />A.2. Purpose of This Evaluation <br />The purpose of this evaluation was to obtain subsurface soil information to help ML Terry <br />Maurer of the City of Elk River design new pavements and prepare plans and specifications for <br />improvements to the roadway. <br /> <br />A.3. Scope <br />The desired scope of services was outlined in a meeting between Mr. Maurer and Mr. Darin <br />Hyatt of our fIrm on October 4, 2004. On October 7, we submitted a proposal to the City of Elk <br />River, which was subsequently authorized by ML Maurer. Our services were provided under the <br />terms of our General Conditions dated March 1, 2003. Our scope of services was limited to: <br /> <br />staking prospective boring locations and coordinating the locating of underground <br />utilities near the boring locations, <br />conducting eighteen penetration test borings to a 5 feet along the existing alignments, <br />classifying the samples and preparing boring logs, <br />analyzing the results of the field tests, <br />formulating recommendations for earthwork and pavements, <br />discussing the results and recommendations with Mr. John Anderson of the City of Elk <br />River, and <br />submitting this geotechnical evaluation report containing logs of the borings, our <br />analysis of the fIeld tests, and recommendations for earthwork and pavements <br />construction. <br /> <br />A.4. Documents Provided <br />Mr. Maurer provided us with a partial map ofthe City of Elk River titled "2005 Proposed Street <br />Reconstruction," indicating the project location. Neither the preparer nor the date it had been <br />drawn was indicated.
