SERVICES-Fax-Back Service: The 24/7 Fax-Back Service provides all Customer employees included within the Coverage(#Locations)identified above,access to a toll-free telephone number in order to request an MSDS and have the requested MSDS supplied via facsimile. Customer is allowed up to the number of fax events listed in the Fax-Back Service column on an annual basis. Any chemical exposure support or medical information calls to the 24/7 Fax-Back Service will be counted as five(5)fax events. Customer has the option to purchase additional fax events in bundles of ten(10)for$200.00. Annual pricing includes an initial allotment of ten(10)8.5 x 11 Posters and fifty(50)1-800-Number Stickers to communicate the program. Additional materials are available upon request at$2.50 per Poster and$0.10 per Sticker(minimum$25 order). All Year One(1)fees(except Compliance Services fees)become due upon Subscription and/or Additional Compliance Solution delivery. Additional years, covering any Subscription, Service&Additional Compliance Solution identified above,and Technical&Customer Support,will be invoiced approximately 45-60 days ahead of the anniversary of the Year One(1)Subscription(s)delivery. Customer may at its discretion pre-pay the full term of the agreement. This Customer Order is governed by the terms and conditions of the MSDSonline Master Subscription Agreement,as posted on www.MSDSonline.com. By signing below,Customer agrees to be bound by such terms and conditions.This Customer Order must be executed within 30 days of the Order Date set forth above.
