City policy on use of social security number. Social security numbers may be requested <br />from individuals only when collection or use of the social security number is mandated by federal <br />or state law or when use of the social security number for purposes of identifying an individual <br />is essential to the performance or operation of City functions that are required by federal or state <br />law. <br />2. Designing forms. All forms used for the collection of data on individuals and existing <br />as of the adoption of this policy have been reviewed for compliance with City policy regarding <br />the use of social security numbers. All forms designed after the date of adoption of this policy, <br />for the purpose of collecting data on individuals, must conform to the City policy on the use of <br />social security numbers. The Responsible Authority is required to review and approve all forms <br />used to collect private or confidential data on individuals. <br />CMS08 B4 <br />GAITS -26 52 <br />
