a. Section A records requester identification information. The name, address and <br />other identifying information cannot be required of an individual requesting public <br />government data (except where copies are to be mailed). Personal identifying <br />information may be required for requests of data classified as other than public. <br />b. Section B records the action taken by the designee. This section is always <br />completed by the Responsible Authority, designee or other authorized employee. <br />The "Authorized Signature" (item 12) should be that of the Responsible Authority <br />or the designee. <br />C. Section C records, when applicable, information pertaining to fees to be charged <br />and collected. See Section 9, "fees for copies of Government Data". <br />Item 14 provides for the calculation of the "Flat Rate". The total amount due is <br />entered, as applicable, in either items 16 or 17. The current "Flat Rate" Is <br />contained in Exhibit H. <br />When the Special Rate is used and is not covered by a Standard Fee Schedule <br />(Section 9.13.), the "Copy Cost Calculation Form", (Exhibit 1) is completed and <br />attached to the "Information Disclosure Request Form". The total charges from <br />the calculation form are entered, as applicable, in items 16 or 17. <br />When the charges are based on a Standard Fee Schedule a copy of the calculation <br />form used to establish the "schedule" need not be attached to the "Information <br />Disclosure Request Form" form unless specifically asked by the requester. <br />d.
