a. the purpose and intended use of the requested private or confidential data within <br />the City; <br />b. whether the individual may refuse or is legally required to supply the requested <br />private or confidential data; <br />C. any known consequences arising from the individual's supplying private or <br />confidential data; or <br />d. any known consequences arising from the individual's refusing to supply private <br />or confidential data; and <br />e. the identity of other persons or entities authorized by state or federal law to <br />receive the data. (Minn. Stat. § 13.04, subd. 2) <br />NOTE: Unless otherwise provided for by law (e.g., education and medical data), the <br />Responsible Authority or designee must provide minors from whom private or confidential <br />data is collected with a notification that the minor individual has the right to request that <br />parental access to the private data be denied. This notification should be in writing and <br />may be contained on the Tennessen Warning. (See Section 10, "Parental Access to Data <br />on Minors") (Minn. Rules, Pt. 1205.0500, subp. 3.A.) <br />NOTE: In accordance with the Federal Privacy Act of 1974, "any Federal, State, or local <br />government agency which requests an individual to disclose his social security account <br />number shall inform that individual whether that disclosure is mandatory or voluntary, by <br />what statutory or other authority such number is solicited and what uses will be made of <br />it." (See Section 14, "Use of the Social Security Number") <br />5. Restrictions on Use of Data. Private and confidential data may not be collected, stored, <br />used or disseminated for any purpose other than those stated to the individual in the Tennessen <br />Warning, except: <br />a. data collected prior to August 1975 may be used for the purposes for which it was <br />originally collected or for new purposes specifically approved by the <br />Commissioner of Administration; <br />K data may be used and disseminated to individuals or agencies specifically <br />authorized access to the data by a state, local or federal law enacted after the data <br />was collected-, <br />C. data for which the Commissioner of Administration has specifically approved a <br />new use or dissemination to carry out a function assigned by law; and <br />d. data for which the individual subject of the data has given his/her informed <br />consent, (See Section 5, "Informed Consent") (Minn.
