Adopt, by motion, a resolution denying a massage therapist license to ChunXing Lin. <br />Or if Council believes the applicant provided enough evidence to meet city code requirements: <br />2. Approve, by motion, a massage therapist license to ChunXing Lin to work at Joy Massage valid <br />through December 31, 2016. <br />Background/Discussion <br />Staff received an application from ChunXing Lin requesting a Massage Therapist License to work at Joy <br />Massage. Staff proceeded to verify the application materials. In the course of our investigation, we found <br />the provided education documents were false. These documents are the official school transcript and the <br />certificate of graduation from the Professional School of Massage in Langhorne, PA. Copies are available <br />in your packet. <br />Per City Code, Council should deny the license request. Two sections that outline reasons for denial are <br />as follows: <br />38-224 (c) (1) Denial, suspension, or revocation <br />If a license application contains any false, fraudulent, or deceptive statements, such statements are <br />grounds for denial of the license. Denial in this case would be based on the untruthful answer by Mr. Lin, <br />who in his application declared the information he provided to be truthful and understood that <br />falsification of answers may result in denial of the license. <br />38-243 Educational Requirements <br />City Code requires an applicant provide proof of graduation, such as a diploma or certificate of <br />graduation. <br />The Arizona State Board of Massage states documents signed by Aifen Zheng for the Professional <br />School of Massage in Langhorne, PA, are not valid. Mr. Lin's certificate is signed by Aifen Zheng. <br />According to Winona Bontrager, President of the Lancaster School of Massage, the documents are false. <br />Ms. Bontrager submitted a letter to the city stating the school has no records of Mr. Lin having graduated <br />P0WIREU 6r <br />Template Updated 4/14 <br />IN <br />
