REQUEST FOR ACCQ'MMODATIQN PROCEDURE <br />C/TY OF ELK RIVER <br />13065 Orono Parkway <br />Elk River, MN 55330 <br />441-7420, TDD 441-7420 <br />This request for accommodation procedure is <br />established to meet the requirements of the <br />~~ <br />Americans With Disabilities Act (ADA). It maybe used by anyone who wishes to <br />request special accommodations. <br />The request for accommodation should include the following information: <br />~ Date of request <br />~ Person requesting action <br />~ Person in need of accommodation <br />~ Telephone number of person making request <br />~ Location of concern <br />~ Identification of issues <br />~ Identification of the requested accommodation <br />~ Time frame requested <br />• PROCEDURE <br />The request should be in writing on a form provided by the city of Elk River. <br />Alternate means of filing a request, such as verbally or by tape recording will <br />be made available for persons with disabilities upon request. <br />2. The request should be submitted 72 hours prior to the need for <br />accommodation. <br />3. The ADA coordinator will review the request to determine if the requested <br />aid or service appropriately meets the individual's needs. <br />4. The ADA coordinator will determine the cost to implement the request and <br />seek budget approval from the assistant city administrator. <br />5. Following budget approval, the ADA coordinator will respond in writing or by <br />telephone, to the person requesting action. <br />6. The ADA coordinator will coordinate the implementation of the <br />auxiliary aid or service; etc. <br />• This document is available in alternate formats upon a 72 hour request.
