® The employee must notify his/her supervisor and complete a leave of <br />absence form ~,ithin ~$ h0l.1rS of rPCPiVing call foT,ury duty <br />o An employee excused or released from jury duty during his/her regular <br />work hours must report to his/her supervisor immediately thereafter. <br />Time spent on jury duty will not count as time worked in computing <br />overtime. <br />49$d 1PERS®NAL LEAVE ®AV <br />Each .lone 1st, every regular employee will be credited with one day of paid personal <br />leave to be taken during the next 12 months with the approval of management. This day, <br />if unused, will not be carried over from year to year. <br />4}:~ ~EREAVEl~E1~I'Q' LEAVE <br />A leave of absence, with pay and fringe benefits may be granted to a regular full-time <br />employee for a maximum of three (3) days in the event of the death of a spouse, child, <br />parent, brother, sister, grandparent, grandchild, or parent-in-law, brother-in-law, sister-in- <br />law, grandparents-in-law, step-parents, step-siblings and step-children. <br />4~:~ GENERAL LEAVE <br />The TT TUt111tieS recognizes that circumstances of a unique personal nature may cause <br />an employee to seek time off without pay. The T TAU ilities may, at its sole discretion, <br />grant unpaid personal leaves of absence to employees ("General Leave"). Approval is <br />required of the employee's immediate supervisor, +'' t, "`*"^°^+ u°ud and the T T <br />^ a-~~~~~*~^*°~Utilities' Director of Operations before General Leave will be granted. In <br />no event will General Leave be granted for longer than ninety (90) days, unless special <br />approval is received from the Utilities' Director of Operations. <br />Vacation and sick leave will not accrue during a General Leave. Further, the employee <br />on General Leave will not be eligible for holiday pay during the leave. The <br />TT T+.~1.'~.~.L1tilltieS will not contribute to the cost of insurance premiums for an employee on <br />General Leave. <br />Employees are expected to return from General Leave when the reason for the leave <br />expires.
