Larson Excavating, Inc. 2255 12th St. SE St. Cloud MN 56304 <br />LOCATION OF WORK: TOTAL SUPPLEMENTAL AGREEMENT AMOUNT <br />Topsoil blending - 38+00 to 74+00 and 300+00 to 308+74 $95,778.00 <br />Steel sheet piling shoring - 56+85 to 57+55 <br />Modular block retaining wall <br />Minnesota School of Business - 5+80 to 7+00 <br />Phis Contract is between the Local Government Entity and Contractor as follows: <br />WHEREAS: This Contract provides for, among other things, construction of a collector <br />roadway and city utilities; and <br />WHEREAS: This Contract further provides for soil retainage by means of steel sheet <br />pilling; and <br />WHEREAS: The Contract provides for topsoil spreading; and <br />WHEREAS: The contractor has provided additional soil boring data showing the <br />conditions differ from those found in the project geotechnical investigation; <br />and <br />WHEREAS: The onsite topsoil material is marginal in organic content and its ability to <br />sustain reasonable growth; and <br />WHEREAS: The conditions differ with the recent construction of the Minnesota school <br />of Business site; and <br />WHEREAS: The Engineer has determined that additional steel sheet piling shoring, <br />Topsoil blending, and additional modular block retaining wall is needed; <br />and <br />WHEREAS: The Engineer has further determined that this constitutes Changed <br />Design. <br />NOW, THEREFORE, IT IS MUTUALLY AGREED AND UNDERSTOOD THAT: <br />1. The Contractor will provide additional steel sheet pile and tie backs as shown in <br />design by Wenzel Engineering Dated 7/6/09 between stations 56+85 and 57+55 <br />2. The contractor will blend the salvage topsoil with the excavated peat material at a <br />ratio of 50% topsoil / 50% peat prior to respreading on all disturbed areas except <br />4+00 to 38+00 <br />
