Further, the undersigned acknowledges that nothing contained in the application shall impose any liability on Elk River Municipal <br />Utilities for the work performed and information presented by the customer's engineer, contractor, or vendor. <br />Customer Signature: <br />.Qualifying Customers <br />This program is applicable to the commercial and industrial customers of Elk River Municipal Utilities only. <br />Date: <br />Rebate Program Guidelines <br />Elk River Municipal Utilities offers cash rebates to qualifying customers who purchase and install qualifying energy-efficient lighting products <br />in new buildings. Illuminating Engineering Society (IES) light levels are recommended. <br />Specific Rules and Qualifications <br />1.Energy-efficient electronic and cathode-disconnect ballasts must meet the following ballast performance criteria: <br />• Power Factor greater than .90 <br />• Total Harmonic Distortion (THD) of less than .20 <br />• Current Crest Factor (CCF) of less than 1.7 <br />• Class "A" sound rating <br />• UL approved, Class 'P' <br />• Three-year manufacturer's equipment warranty <br />• Ballast factor greater than .7 <br />2. Compact Fluorescent Lamps must meet the following minimum criteria: <br />• Power factor greater than .90 <br />•Total Harmonic Distortion less than .35 <br />Warranty Information <br />Rebate qualifications do not imply any representation or warranty of such equipment, design, or installation by Elk River Municipal Utilities. <br />Elk River Municipal Utilities shall not be responsible or liable for any personal injury or property damage caused by this equipment. Elk River <br />Municipal Utilities does not guarantee that a specific level of energy or cost savings will result from the implementation of energy <br />conservation measures or the use of products funded under this program. In no event shall Elk River Municipal Utilities be liable for any <br />incidental or consequential damages. <br />Other Important Program Rules <br />1. Installation must be complete before funds will be issued for the rebate. <br />2. Customers and vendors must submit itemized equipment invoices along with rebate applications and worksheets to Elk River Municipal <br />Utilities. To ensure that the equipment installed meets Elk River Municipal Utilities performance standards, these invoices must itemize labor <br />charges, quantity and price of the equipment installed, and information regarding the manufacturer and model numbers for all lamps, <br />ballasts and fixtures included-in the rebate. <br />3. Elk River Municipal Utilities will conduct inspections of installations. Call 763/635-1330 to schedule an inspection. <br />4. The customer is responsible for checking with Elk River Municipal Utilities to determine whether funding is available and to verify program <br />parameters. <br />5. The maximum rebate amount shall be limited to 50% of the project cost up to $10,000. <br />6.
