IV. <br /> <br />Vo <br /> <br />This License will take effect when all of the conditions precedent to the issuance of the <br />License have been met and Licensee, or its duly authorized representative, has executed <br />this License indicating Licensee's consent to all of the terms and conditions of this <br />License, the Conditional Use Permit for the Facility, and Section 604 of the City Code. <br /> <br />In no event shall operations at the Facility commence until all of the terms, conditions, <br />and requirements contained in or incorporated into this Licensee have been met, including <br />all required tests, reports, and inspections. <br /> <br />Dated: By: <br /> <br />Hank A.
