(Liesch) is submitting this letter and accompanying Conditional Use Permit <br />(CUP) Application submittal package (the Application} on behalf of Jason Bebeau (J & B Mining) <br />for the above referenced Properly. Figure 1 shows the location of the Property and Figure 2 shows <br />the Existing Conditions, as of Apri12004. <br />The following documents aze included with this letter and complete the Application: <br />1. Completed CUP Application signed by the applicant, including the application fee of <br />$520.00 and consultant retainer fee of $1000.00; <br />2. <br />3. Plan Sheets 1 through 3: Existing Conditions Plan, Mining Area Grading Plan, Drainage <br />and Restoration Plan prepared by Liesch Associates, Inc., dated May 27, 2004; <br />4. Storm Water Pollution Prevention Plan, Schmidt Gravel Mine, Elk River, Minnesota, <br />prepared by Liesch Associates, Inc., dated May 2004; <br />5. Labels for property owners within 350 feet of the Properly. <br />6. Rezoning; <br />7. EAW; <br />8. Environmental Spill Plan; <br />Liesch submitted a Jurisdictional Wetland Identification and Wetland Delineation for the subject <br />property to the City of Elk River on May 17, 2004. <br />A completed Part 1: Basic Application and Part 2: Replacement Plan Supplement Minnesota. <br />LocaUState/Federal Application Form for Water/Wetland Projects, signed by the applicant; and <br />accompanying plan sheets are being submitted by Anderson Engineering of Minnesota, LLC under <br />separate cover. <br />
