Please answer the questions as completely as you can. Remember that <br />this evaluation is based on your attached position description. All position descriptions were <br />reviewed in March and should accurately reflect a summary of the duties performed. <br />Include many examples for each question. The information you provide will assist in the <br />review of your appeal.
