Please feel encouraged to call with questions pertaining to the <br />application. <br /> <br />PROPOSAL CHECKLIST <br /> <br />o Cover sheet (attached) <br />o Proposal narrative (see above) <br />o Financials, including: <br />I. Financial statement from most recently completed year, audited if available, showing <br />actual expenses. This information should include a balance sheet, a statement of <br />activities (or statement of income and expenses) and functional expenses. As an <br />alternative for an audited statement you may submit a recent Form 990 tax return. <br />2. Project Budget, including income, in-kind donations, and all expenses. Please note that <br />our ability to support Healthy Lakes/Rivers Partnership Projects is limited to $5,000. <br />(see below) <br />o Completed Grantee Action Plan & Evaluation form (attached) <br />o Board list with affiliations <br />o Brief description of key staff, including qualifications relevant to the specific request <br />o IRS Determination 501(c)3 Letter (not applicable for government) <br />o Names and addresses of any consultants being used <br />o Governing body minutes or letter authorizing application <br />o Letters of support (not required) <br />o An original and one copy of grant proposal (unbound) <br />
