<br />EXHIBIT B <br /> <br />Waiver <br /> <br />All employees who use the fitness room and equipment, and all Quests, must sign <br />a release waiver prior to using the fitness room. This is a requirement of the City <br />insurance company. <br /> <br />A vailable Times <br /> <br />The City fitness room and equipment will be available to all regular, full time and <br />part time employees, the Police Reserves, Volunteer Fire Department, and City <br />Council. The room is open 24 hours a day, but Quests are onlv allowed with an <br />emolovee. <br /> <br />Lockers and Showers <br /> <br />Lockers and showers are available in the exercise room vicinity. The Police <br />Department employees and Police Reserves are to use the lockers and showers <br />located in the police department; all other employees use the rooms located in the <br />fitness room. <br /> <br />Since these locker rooms are shared, employees are required to leave the locker <br />rooms in a clean, neat condition and do not leave personal items in the lockers. <br />Employees should be courteous - keep your time limited in the locker rooms if <br />others are waiting. <br /> <br />The City will not be providing towels for the locker room or for use when <br />employees are using the equipment. <br /> <br />Miscellaneous Issues <br /> <br />Training on the use of the equipment can be arranged in group settings as <br />necessary . <br /> <br />Keys will be provided to all employees who desire access to the room. <br /> <br />Disinfectant spray and a towel are located in the fitness room to clean equipment <br />after use. <br />
