discipline up to and including termination. <br />Where safety equipment is required by federal, state, or local rules and regulations, it is a <br />condition of employment that such equipment be worn by the employee. <br />Supervisors are authorized to send an employee home immediately when the employee's <br />behavior violates the city's personnel policies, department policies, or creates a potential <br />health or safety issue for the employee or others. <br />Safety Equipment Reimbursement <br />The city reimburses employees for certain foot and eye protection required by the city <br />safety policy. Employees required to wear protective footwear may be reimbursed based <br />on the Foot Protection Policy attached as Exhibit B. Employees required to wear eye <br />protection may be reimbursed based on the Prescription Safety Eyewear Policy attached <br />as Exhibit C. <br />Incident Reporting <br />Employees are required to report accidents resulting in personal injuries and/or vehicle, <br />equipment, or property damage to their supervisor immediately. <br />Employees shall immediately report all accidents and injuries occurring within the course <br />of employment to their supervisor and complete an Employee Incident Report. The <br />supervisor shall submit a Supervisor's Report of Injury Form to Human Resources within <br />twenty-four (24) hours of the injury. Both forms are on Access Elk River. <br />Employees shall immediately report all damage to city property to their supervisor. The <br />supervisor shall submit documentation regarding the incident to the Finance Department <br />within twenty-four (24) hours of the time of the damage or accident. A copy of the <br />Minnesota Motor Vehicle Accident Report must be submitted for all vehicle accidents. <br />Further details on this policy and a complete summary of responsibilities and procedures <br />are available from the Safety Coordinator. <br />Workers' Compensation <br />An employee temporarily unable to work due to an injury or illness sustained in the <br />performance of their city work may be eligible for Workers' Compensation subject to the <br />provisions of the State of Minnesota Workers' Compensation Law. <br />An employee who becomes eligible to receive Workers' Compensation will retain the total <br />amount of the Workers' Compensation check and may receive the difference between <br />their Workers' Compensation payment and the employee's regular gross salary with <br />accrued sick leave. If accrued sick leave is not available, vacation or compensatory time <br />may be used. The total of the Workers' Compensation check and the accrued leave <br />compensation may not exceed the employee's normal gross pay. <br />Page 60181 <br />Page 214 of 447 <br />
