o Grandchild or grandparent <br />o In-laws (including son, daughter, father, or mother) <br />o Anyone close to you who depends on you like family, even if not related by <br />blood <br />• A serious health condition means a physical or mental illness, ingury, impairment <br />condition, or substance use disorder. Taking care of yourself for this serious <br />condition may involve evaluation, treatment, inpatient care, recovery, or not being <br />able to perform regular work, attend school, or do regular daily activities. This <br />includes childbirth, conditions related to pregnancy, or surgery. <br />Notice <br />Prior to starting a claim with MetLife, employees need to notify their supervisor of their <br />intent to take leave and complete a Leave Request, which can be found on Access Elk <br />River. If the need for leave is foreseeable, employees must provide at least 30 days' notice <br />prior to taking leave. The employee must notify the supervisor and human resources as <br />soon as practicable if the dates of scheduled leave change, are extended, or were initially <br />unknown. <br />If the leave is not foreseeable you will still be able to take leave under MNPL and we ask <br />that you provide as much notice as possible. <br />How to Apply for Minnesota Paid Leave <br />After the above Notice requirement has been met, you may apply for MNPL through our <br />third -party administrator MetLife. (Notifying your employer first is a requirement of MNPL <br />law.) Instructions on filing a claim can be found on Access Elk River or on your Benefits & <br />HR Info page in UKG. <br />MNPL will run concurrently with any leave and/or wage supplement for which you may be <br />eligible for under local, state, or federal law, or city policy which may include: Family and <br />Medical Leave Act (FMLA), Minnesota Women's Economic Security Act (WESA) pregnancy <br />and parenting leave, and Workers' Compensation. <br />The city offers short-term disability (STD) and long-term disability (LTD) that may run <br />concurrently and require their own filing requirement pursuant to the terms of the STD <br />and LTD policies. Please see the applicable plan policy for more information. STD and LTD <br />payments may be reduced, pursuant to the terms of their policy, as a result of receiving <br />MNPL. <br />Supplementing MNPL Benefits with Accrued Paid Leave <br />If you are receiving MNPL benefits, the city allows you to supplement, or "top off," your <br />MNPL benefits with any accrued but unused paid leave.
