3.0 USER PROVIDED INFORMATION <br />To meet the requirement of ASTM E 1527-21, the User is required to provide specific information <br />as it pertains to the Property. The following section provides a brief discussion of User provided <br />information. The User Questionnaire was completed by Mr. Matt Barker with Capstone Homes, Inc. <br />Supporting documents are included in Appendix C. <br />User Provided Information REC?Comments <br />Owner, Property Manager and <br />Occupant Information <br />No Erwin T. and Doris I.
