Premiums and employee <br />shares are subject to change from time to time. <br />COVERAGE MONTHLY EMPLOYEE SHARE <br />• Employee $109 <br />• Employee + 1 $218 <br />• Family $328 <br />Employees should consult the applicable Summary Plan Description (SPD) for details <br />regarding coverage and eligibility. A copy of the SPD will be provided to employees and <br />is available upon request. <br />48. DENTAL INSURANCE <br />Eligible employees who regularly work at least 30 hours per week and who enroll in the <br />Utilities' dental insurance plan, the Utilities will pay seventy-five percent (75%) of the <br />monthly premium per employee/family for dental insurance. Any monthly premium over <br />and above 75% of the dental coverage is the responsibility of the employee. Employees <br />should consult the Summary Plan Description for details regarding coverage and eligibility. <br />A copy of this description will be provided to employees and is available upon request. <br />49. LONG-TERM DISABILITY <br />A Long -Term Disability Plan is provided to eligible employees who regularly work at least <br />30 hours per week. Schedule amount: 60% of Monthly Earnings subject to a maximum <br />amount of $5,000.00 per month. Employees must be employed for two (2) months to <br />qualify for coverage. Employees should consult the Summary Plan Description for details <br />regarding coverage and eligibility. A copy of this description will be provided to <br />employees and is available upon request. <br />50. LIFE INSURANCE <br />Life insurance is provided to each employee at a rate of Two and One Half times (21/2) the <br />employee's annual salary, to a maximum of $85,000.00. Employees should consult the <br />Summary Plan Description for details regarding coverage and eligibility. A copy of this <br />description will be provided to employees and is available upon request. <br />51. SHORT TERM DISABILITY INSURANCE <br />A Short -Term Disability Plan is provided to eligible employees who regularly work at <br />least 30 hours per week. Scheduled benefit amount: 60% of weekly earnings subject to a <br />maximum amount of $1,000.00 per week.
