any Utilities manager or Director, the Human Resources Representative, or the relevant <br />public safety authority. <br />Employees are required to report accidents resulting in personal injuries and/or vehicle, <br />equipment, or property damage to their supervisor immediately. <br />Personal Injuries. Immediately report to your supervisor all accidents <br />and injuries occurring within the course of your employment. The <br />supervisor shall submit a First Report of Injury and a Supervisor's <br />Report of Injury Form to the Human Resources Representative within <br />twenty-four (24) hours of receiving such report from you. <br />Vehicle, equipment, or property damage accidents. Immediately report <br />to your supervisor all damage to Utilities property. The supervisor <br />should submit documentation regarding the incident to the Utilities' <br />Finance Manager within twenty-four (24) hours of the time of the <br />damage or accident. A copy of the Minnesota Motor Vehicle Accident <br />Report must be submitted for all vehicle accidents. <br />The Utilities will provide necessary safety equipment to employees. This may include: <br />• One pair of safety glasses at the time of employment. The employee shall <br />provide the correct prescription for the safety glasses. <br />• The Utilities will pay the cost of new safety lenses, upon a change in the <br />prescription. If needed, frames will be replaced as needed contingent upon <br />supervisor approval. <br />• Safety glasses that are broken or damaged while the employee is on the job will <br />be replaced by the Utilities. <br />13. COMMITMENT TO A DRUG AND ALCOHOL -FREE WORKPLACE <br />The Utilities is committed to providing a safe, healthy, and productive drug and alcohol - <br />free workplace. <br />To promote this goal, employees and others are required to report to work in appropriate <br />mental and physical condition to perform their jobs in a satisfactory manner. Being under <br />the influence of alcohol or illegal drugs (as classified under federal, state, or local laws), <br />including marijuana, while on the job poses serious health and safety risks to employees <br />and members of the public/others, which is not tolerated. As a condition of employment <br />employees must adhere to the terms and conditions of this policy. <br />Prohibited Conduct <br />The Utilities expressly prohibits the following activities at any time that the employee or <br />other person is either (1) on the Utilities' facilities (whether or not the employee is working) <br />or (2) on duty or conducting Utilities business (either on or away from the Utilities' <br />12 <br />June 2023 <br />61847929v4 <br />•A <br />
