DENTAL INSURANCE <br /> <br />Eligible employees who regularly work at least 30 hours per week and who enroll in the <br />Utilities’ dental insurance plan, the Utilities will pay seventy-five percent (75%) of the <br />monthly premium per employee/family for dental insurance. Any monthly premium over <br />and above 75% of the dental coverage is the responsibility of the employee. Employees <br />should consult the Summary Plan Description for details regarding coverage and eligibility. <br />A copy of this description will be provided to employees and is available upon request. <br /> <br /> <br />49. LONG-TERM DISABILITY <br /> <br />A Long-Term Disability Plan is provided to eligible employees who regularly work at least <br />30 hours per week. Schedule amount: 60% of Monthly Earnings subject to a maximum <br />amount of $5,000.00 per month. Employees must be employed for two (2) months to <br />qualify for coverage. Employees should consult the Summary Plan Description for details <br />regarding coverage and eligibility. LIFE INSURANCE <br /> <br />Life insurance is provided to each employee at a rate of Two and One Half times (2½) the <br />employee’s annual salary, to a maximum of $85,000.00. VISION INSURANCE <br /> <br />Eligible employees who regularly work at least 30 hours per week are eligible to enroll in <br />the Utilities’ vison insurance plan.
