<br />Minnesota Opioids Settlement Checklist <br />Cities and counties must complete the following steps: <br /> Register your city or county on the national settlement website: <br />www.nationalopioidsettlement.com. <br />a.Notice with a unique registration code was sent to cities and counties in late <br />September. If your city or county did not receive this notice or cannot find its <br />unique registration code and wishes to participate in the settlements, contact the <br />Attorney GeneralÓs Office. <br />b. Once registered, your designated contact will receive settlement participation <br />packets, including two (2) Subdivision Settlement Participation Forms Î one for <br />each of the Distributors and Janssen (Johnson & Johnson) settlements. The <br />settlement sign-on forms can be completed electronically via DocuSign. <br /> Adopt a county board or city council resolution authorizing a representative of the <br />subdivision to execute the following: <br />a. The Minnesota Opioids State-Subdivision Memorandum of Agreement (MN <br />MOA) <br />b. The Distributor Subdivision Settlement Participation Form <br />c. The Janssen Subdivision Settlement Participation Form <br /> Have the authorized representative execute the following documents: <br />a. The MN MOA <br />b.The Distributor Subdivision Settlement Participation Form (via DocuSign) <br />c. The Janssen Subdivision Settlement Participation Form (via DocuSign) <br /> Return the following documents to the Attorney GeneralÓs Office by email to <br />opioids@ag.state.mn.us: <br />a. Copy of the completed resolution passed by your city or county <br />b. Executed signature page for the MN MOA <br /> <br />Additional information about the settlements and how they are implemented in Minnesota can be <br />found on the Attorney GeneralÓs website: www.ag.state.mn.us/opioids. Subdivisions that are <br />represented by an attorney with respect to opioid claims should consult with their attorney. <br />Additionally, specific questions for the Attorney GeneralÓs Office can be emailed to <br />opioids@ag.state.mn.us, or left via voicemail at (612) 429-7126. <br />
