/511 Pullman Place Cooperative Request to Male Upgrades and. IMprovements — Within Dwelling Unit x undersigned hereby requests the consent of the Board of Directors of Pullman Place Cooperative ("the Cooperative") to upgrade certain items -which constitute improvements to, or appliances in the dwelling unit within the Cooperative which will be, or is, occupied by the undersigned, or which constitute fixtures to, or Permanent improvements to, such unit, which upgrades, appliances and improvements are listed below. The undersigned hereby agrees to the following: l . That such upgrades, appliances and improvements shall become the property of the Cooperative upon installation in the unit, 2. The undersigned shall be responsible for the cost of replacement and/or repair to improvements to the extent that the cost for replacing the items exceed the cost of"standard" items under the Cooperative's Replacement Policy. The cost of repairs for appliances and fixtures other than the brand or brands designated by the Board as being the Cooperative's standard shall be the responsibility of the member. • The undersigned shall, upon the request of the Cooperative, remove such upgrades, appliances and improvements when the undersigned terminates his/her interest in the Cooperative, and shall, at the undersigned's cost, repair any damage to the dwelling unit caused by such removal aad, where applicable and at the undersigned's cost, replace' such upgraded items, appliances and improvements with "standard" items as directed by the Cooperative; and This Agreement shall be bm" ding upon the undersigned and his/her personal representatives, heirs, trustees, guardians, conservators, successors and assigns.
