Leave Benefits <br />The city provides leaves of absence according to the following policies. A Request -for Leave ofAbsence form <br />must be completed for foreseeable leaves other than vacation. Forms are available on Access Elk River or <br />from Human Resources. When possible, advance notice of a leave should be provided to the employee's <br />department director so work schedules can be adjusted accordingly. <br />Depending upon employee's situation, more than one form of leave may apply during the same <br />~i time -period (e.g., the Family and Medical Leave Act is likely to apply during a workers' <br />compensation absence). An employee will need to meet the requirements of each form of leave <br />separately. Leave requests will be evaluated on a case -by -case basis. <br />Employees may record worked hours or use only one type of paid leave for each scheduled work hour. <br />Except as otherwise stated, all paid time off taken under any of the city's leave programs must be taken <br />consecutively, with no intervening unpaid leave. The city will provide employees with time away from <br />work as required by state or federal statutes if there are requirements for such time off that are not <br />described in the personnel policies. <br />Vacation Leave <br />Paid vacation leave is provided for regular, full-time employees and on a prorated basis for eligible part- <br />time employees. To be eligible, part-time employees must work a set schedule with a minimum of forty <br />(40) hours per biweekly pay period. Most Ttemporary-seasonal, and part-time employees are not <br />eligible for vacation benefits. <br />Vacation leave is accrued based on consecutive years of leave -eligible service. o <br />determiner�g an employee's vacation accrual rate, years of service will include all continuous time the <br />employee has worked at the city in a position that is eligible for paid leave (including authorized unpaid <br />leave). Employees who are rehired after terminating city employment will not receive credit for their prior <br />service unless specifically negotiated at the time of hire.
