EXHIBIT A <br />City of Elk River <br />Fitness Room Policy <br />Iz110:t•1911144010121 <br />The City of Elk River strongly believes in a philosophy of promoting employee wellness. An employee <br />wellness program benefits both the employer and employee as well as the community. Healthy employees <br />tend to be more productive and healthy employees reduce expenditures (i.e. city insurance costs; use of <br />sick time and its related overtime expenses; etc.). <br />In the spirit of a healthy workplace, the city provides and outfits a fitness room as one part of a greater <br />city wellness program. <br />POLICY <br />This policy encourages everyone to utilize the wellness room; with respect to limitations of types of <br />equipment, locker space and workout space. <br />Employee and Guest <br />The fitness room is not a health club. Use of this room and equipment is open to all city and municipal <br />utilities employees, and city council Members. Employees may bring one guest at any one time. Guests <br />must be over the age of eighteen (18). Due to safety and insurance issues, no one under the age of <br />eighteen (18) is allowed in the fitness room under any circumstances. <br />1A7=— N <br />All employees who use the fitness room and equipment, and all guests, must sign a release waiver prior to <br />using the fitness room. This is a requirement of the city insurance company. <br />Available Times <br />The fitness room is open 24-hours a day, but guests are only allowed with an employee. <br />Lockers and Showers <br />Lockers and showers are available. Lockers are intended to secure street clothes and valuables while <br />exercising, not as an extended use. Employees shall leave the locker rooms in a clean, neat condition and <br />shall not leave personal items in the lockers. Employees should be courteous - keep your time limited in <br />the locker rooms if others are waiting. <br />
