<br /> <br />Appendix A <br /> <br />Required Financial Information Required With Application <br />• Detailed Sources and Uses of funds <br />o Identify and isolate any extraordinary redevelopment and/or clean-up <br />expenditures <br />o For mixed-use projects: separate commercial and residential costs <br />• Revenue/Income Projections <br />o Residential: Include monthly unit rent, unit count, and square footage for <br />each unit type. Also include any other income (Parking, Storage, Pet <br />Fees, etc.). For low income housing tax credit projects, include the full <br />housing workbook submitted to the state housing finance agency. For <br />senior care facilities, breakout the services as separate income. <br />o Commercial: Include square footage of each leasable space and the per <br />square foot lease rate for each space. Indicate the terms of leases <br />(duration, N, NNN, etc.) Also, include the expense on Commercial Space <br />(Property Taxes and CAM). <br />o Hotel: Include average daily rate, occupancy percentages, REVPAR and <br />TREVPAR. Include any additional revenue from restaurant, bar and/or <br />banquet operations, and any misc. revenue (vending, etc.). <br />o All: Include vacancy rate projections <br />• Detailed Operating Costs <br />o Include the major categories, such as administrative, payroll, utilities, <br />insurance, maintenance, management fees, property taxes, etc. <br />• Financing Assumptions <br />o Rate <br />o Amortization <br />o Term <br />o Underwriting requirements (DCR, LTV, etc.) <br />• 15-Year Operating Proforma <br />• Site Plan and location map <br />• Building Elevations <br />• For residential projects: <br />o Detailed list of amenities <br />o Types of parking (Underground, structured, and/or surface) <br />o Interior unit finish materials <br />o Exterior finish materials <br />• Market and/or Feasibility Study <br /> <br />Significant additional information may be requested at any time by the Ci ty/EDA and <br />may be in addition to the materials outlined in this application. The Applicant shall be <br />required to submit any and all information as requested by the City/EDA.
